Greater Form Customization

It would be great to be able to build custom forms for mobile intake to supplement the current forms—although I do recognize that the original forms help with tracking statistics! For example, NIFLA suggests having a space for the client to write names of those invited to the ultrasound in their Request for Medical Services, and they have slightly different questions on the health history. This would also allow me to ask clients custom questions related to faith. Thank you.
